Understanding lymphoedema
What is lymphoedema?
Lymphoedema is long-term swelling that develops when the lymphatic system cannot drain fluid from the body’s tissues as well as it should. Fluid builds up under the skin, most often in an arm or a leg, though it can affect other areas.
It usually develops when lymph nodes or lymph vessels have been damaged or removed — for example after surgery, radiotherapy or as part of cancer treatment. It can also follow infection or injury, and in less common cases it can be present from birth or develop without an obvious cause.
Early on, the swelling may come and go, and the limb may feel heavy, tight or full. Over time, if it isn’t managed, the swelling can become more persistent and the skin and underlying tissue can change in texture.
Lymphoedema is generally a long-term condition. It is not usually cured, but with appropriate management it can often be controlled well.
Not all swelling is lymphoedema.
Swelling has many possible causes. New, sudden, painful, red or unexplained swelling should be assessed by a doctor before starting any lymphoedema treatment.

Manual lymphatic drainage
What is Manual Lymphatic Drainage?
Manual lymphatic drainage — often searched for as lymphatic drainage massage, lymphatic massage or simply MLD — is a specific hands-on technique that uses very light, rhythmic movements applied to the skin in a particular sequence and direction. The aim is to encourage lymph fluid to move along pathways that are still working well.
It is not a firm or deep massage. The pressure is deliberately gentle, and the sequence matters as much as the technique itself.
In lymphoedema care, manual lymphatic drainage is one possible component of an individualised management plan — not a treatment on its own, and not something that suits every person or every presentation. Whether it is appropriate, and how it fits alongside exercise, skin care, compression and self-management, depends on your assessment.
Lymphatic massage and lymphoedema therapy are not always the same thing.
A relaxation or wellness lymphatic massage is a different service with different goals. Lymphoedema management is a clinical process that starts with assessment and is planned around your individual condition.

Every plan is different. Depending on your assessment and your needs, treatment may include some of the following.

Your first visit starts with understanding your history, looking at the affected area and taking measurements where appropriate. This is what everything else is planned around.

Manual lymphatic drainage
Where appropriate, gentle hands-on techniques may be used to encourage lymph movement, as one part of a wider plan rather than a treatment on its own.

Movement and exercise
Specific, graded movement is often part of lymphoedema management. Anything suggested will be matched to what you can comfortably do.

Compression and positioning
Where suitable, we'll discuss how compression garments and everyday positioning fit into your routine, and how to keep using them consistently.

Most of lymphoedema management happens between appointments. A large part of each visit is making sure you and your family know what to do at home.
Not everyone receives every component. Your plan depends on your assessment, your condition and your goals.
